submit to Carbon County Clerk - 636-3200 - 120 East Main Street

(for verification clerk may require identification)

Return application to Price City Library

Carbon County pays the $15.00 fee

If drivers license or Ut ID Card does not show current address

then one of the following must be shown in addition to:

Rental or lease agreement

(Typed - No written receipts allowed)

Utility Bill

Mail receipt from a government entity

Printed checking or savings accounts
