Price City Residents (Within Price City Limits)
Complete application.
Make sure both the application and rules are
signed and submit to the librarian.
No fee if living inside Price City limits
-Must present acceptable form of identification*
showing proof of residency (street address)

Carbon County Residents in Non-incorporated Areas 
(Not within any city limits)
Complete application.
Make sure both the application and rules are
signed and submit to the librarian.
If drivers license or UT ID Card does not show current address then one of the following must be shown in addition to above:
Rental or lease agreement
(Typed - No written receipts allowed)
Utility Bill
Mail receipt from a government entity
Printed checking or savings accounts

Emery County, Helper, Wellington, East Carbon & Sunnyside Residents
Fill out application
$15.00 fee per year
Must present acceptable form of identification*
showing proof of residency (street address)

 
College of Eastern Utah Students
Students living in the dorms are required
to follow interlibrary loan procedures.
Inquire at desk.
